Accreditation Statement

This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the University of Wisconsin School of Medicine and Public Health, the ASPI, and the Massachusetts Pain Initiative.  The University of Wisconsin School of Medicine and Public Health is accredited by the ACCME to provide continuing medical education for physicians. 

Credit Designation Statement

The University of Wisconsin School of Medicine and Public Health designates this educational activity for a maximum of 8.75 AMA PRA Category 1 Credits™.  Physicians should only claim credit commensurate with the extent of their participation in the activity.

Continuing Education Units

The University of Wisconsin-Madison, as a member of the University Continuing Education Association (UCEA), authorizes this program for 0.875 continuing education units (CEUs) or 8.75 hours.

You will be mailed a conference completion report approximately 4 weeks after the conference.

Policy on Faculty and Sponsor Disclosure

As a sponsor accredited by the ACCME, it is the policy of the University of Wisconsin School of Medicine and Public Health to require disclosure of the existence of any significant financial interest or any other relationship a faculty member or a sponsor has with either the commercial supporter(s) of this activity or the manufacturer(s) of any commercial product(s) discussed in an educational presentation.  Detailed disclosure will be made in the course handout materials.

Nursing Continuing Education

The University of Wisconsin-Madison Continuing Education in Nursing is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.

This activity has been planned and implemented through the joint sponsorship of the University of Wisconsin-Madison Continuing Education in Nursing and other sponsors. 8.75 ANCC Contact Hours will be awarded for successful completion of this entire educational activity. For Iowa credit, the award is 10.5 Contact Hours. Verification of successful completion is based on attendance at all sessions with satisfactory completion of all evaluation materials.

Pharmacy Continuing Education

Extension Services in Pharmacy at the University of Wisconsin-Madison School of Pharmacy is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education.  Attendees may earn up to 8 hours of continuing education credit (0.8 CEUs).  Verification of CEUs is based on attendance at lectures and workshops, with satisfactory completion of all evaluation materials.  Statements of credit will be mailed by Extension Services in Pharmacy within one month of receipt of evaluation materials.  Universal Program #073-999-07-065-L01.

Social Work Continuing Education

This program is approved by the National Association of Social Workers (provider # 886474141) for 8 continuing education contact hours.
